Role Overview
The Medical Records Intake Specialist serves as the first checkpoint in the medical records retrieval process. This role ensures that incoming requests are accurate, complete, and compliant before being released to downstream teams.
Accuracy at intake directly affects turnaround time, provider acceptance, and overall client satisfaction. This role requires focus, consistency, and comfort working with structured documentation daily.
Key Responsibilities
Request Review & Validation
- Review incoming ROI forms and medical record requests for completeness
- Validate patient identifiers, provider details, dates, and signatures
- Confirm request accuracy against internal intake standards
Document Preparation
- Correct minor errors in forms when permitted
- Identify missing or conflicting information that could delay retrieval
- Prepare clean, standardized request packets
System Entry & Tracking
- Enter request data accurately into internal systems
- Assign request identifiers and track intake status
- Ensure documentation is correctly labeled and attached
Quality & Compliance
- Follow HIPAA-compliant handling procedures
- Apply SOPs consistently across all requests
- Escalate unclear or non-compliant requests

- High first-pass acceptance rate
